Theft Insurance Claim Process

  • Step 1 Call Toll free Customer Care (1800-209-9060) and inform Theft within 48hrs.

  • Step 2 Get "Claim Intimation Number" (CIN) Registered

  • Step 3 Intimate Police Station on Theft and get Police Intimation letter "duly stamped & signed". Intimation letter to be procured within 96 hours of the incident, failing which insurer reserves right of claim process on non standard basis.

  • Step 4 Block SIM Card with your Telecom Operator and get SIM Blocking Letter "duly stamped & signed". SIM Blocking Letter to be procured within 96 hours of the incident, failing which insurer reserves right of claim process on non standard basis.

  • Step 5 Courier the documents to UB Insurance Associates, Bangalore at the address mentioned on top left of this page

Documents Required:

  1. 1. Covering Letter
  2. 2. Original Bill on Claimant's Name
  3. 3. Self Attested Photo ID Proof of Claimant
  4. 4. Police Intimation letter "duly stamped & signed": Intimation letter to be procured within 96 hours of the incident, failing which insurer reserves right of claim process on non standard basis
  5. 5. SIM Blocking Letter or Email Copy from Service Provider "duly stamped & signed": SIM Blocking Letter to be procured within 96 hours of the incident, failing which insurer reserves right of claim process on non standard basis
  6. 6. If claimant is not a SIM owner, SIM owner photo ID proof to be submitted with rest of the documents
  7. 7. Itemised Bill for Postpaid Customers
  8. 8. Insurance Claim Form (* marked are all mandatory information).
  9. 9. Claim Pre-Receipt Voucher - Bank Details Form (* marked are all mandatory information).
  10. 10. Cancelled Cheque showing your account details (Including IFSC Code and MICR Code).
  11. 11. Letter of Subrogation on minimum Rs.20 Stamp Paper along with Notary.

Bank Details of Insured (Of Claimant's Name):

  1. 1. Account Name (as per Bank's record)
  2. 2. Bank Name & Branch Address
  3. 3. Savings Account Number
  4. 4. MICR CODE
  5. 5. IFSC CODE
  6. 6. Cancelled Cheque showing your account details (Including IFSC Code and MICR Code).

CLAIM SETTLEMENT TAT (After Documents Submission): 21 Working Days (14 Days for Insurance Status + 7 Days for Money Transfer)

Note: For timely processing and settlement of your claim you are advised to send "complete set of documents duly filled and signed" within 5 days of generating the claim intimation number.
